“Your child has already turned in a completed sports physical. Our district has decided to utilize PlanetHS for the 2019-2020 school year. Because of this change, you will need to follow the steps of the attachment below and create accounts (1 for the parent and 1 for the student). Once you have created an account and completed the forms online, I will add you sports physical to the account.
